Original Abstract
Introduction: Continuous positive airway pressure (CPAP) remains to be the most effective therapy for obstructive sleep apnoea syndrome (OSAS). Long-term adherence is extremely important. The aim of this study was to assess ten-year adherence to CPAP and to define factors influencing it. Methods: All subjects involved were consecutive patients of Sleep Laboratory of Department of Respiratory Diseases, Olomouc. Anthropometric data, night sleep testing and detailed history have been completed. The data about CPAP adherence have been obtained through device memory. Subjects who completely refused the therapy have been assigned to the group A, and subjects with good CPAP adherence (with 4 or more hours use per night in average) were assigned to group B and followed-up for 10 years. Patients who died during the follow-up and those with CPAP usage less than 4 hours per night have been excluded. To analyse data, we used standard statistical tools and predictive machine learning tools. Statistical significance has been evaluated with alpha level 0.05. Results: Group A consisted of 106 individuals and group B of 166 subjects. Factors with impact on CPAP adherence were age, sleepiness, microsleep, fibrosis, smoking, AHI (apnea-hypopnea index) and mean night saturation. In total seven predictive machine learning algorithms have been examined together with its statistical evaluation. Several interesting frequent patterns were identified, e.g., people of age ≥68 and AHI ≥37.8 cm typically have good adherence. Conclusion: Good adherence to CPAP therapy in OSAS patients is essential. According to our results, higher age and OSAS severity predicts higher CPAP usage. Obesity, CAD and fibrosis predicts poor CPAP usage. Machine learging may help to found new predictors of CPAP adherence.
