Original Abstract
The precise magnetic resonance images of real subjects revealed large variability of the heart position in relation to the ecg lead V2 placed in the 4th intercostal level and considered as an anatomical reference. The use of standard torso model in which the heart location is modeled with respect to V2 thus cannot reflect its actual position sufficiently. In this study the method for assessment of vertical heart position in standard torso model, corresponding to the actual heart position in an observed subject is presented. The location of the initial activity during ventricular depolarization was suggested as the representative of the position of left ventricular septum. Assumption that small activated volume of septum can be represented by one dipole was used to find the point of initial ventricular depolarization by searching a dipolar generator by an inverse solution from surface ecg potential maps. In the study 25 subjects with well documented individual torso geometry obtained by MR were analyzed. A reference point in septum was assigned to each individual heart model and also to the heart in the standard torso model. For all subjects surface potentials were measured in 62 leads and integral maps computed for time intervals within the first 20 ms of the QRS complex were used to compute the inverse solution in homogeneous torso model. For each subject the heart in the standard torso model was then moved to a new position so that the reference point corresponded to the inversely estimated point of initial ventricular depolarization. The standard torso models with individually moved heart positions better corresponded to the real torso geometry. Even better results were achieved for 3 selected subjects in which also dimensions of the standard torso model were adapted and electrodes were placed in concordance with the real torso before searching the heart position.
