Original Abstract
Background: Irreversible electroporation (IRE) holds the promise to revolutionize catheter ablation of arrhythmia, overcoming the limits of thermal-based therapies. A careful combination of pulse parameters, safety conditions and knowledge of cell death mechanism must be pursued to maximize the benefits of the envisioned therapies. Aims: Identify a combination of parameters to induce complete local cell death. Prove the feasibility, efficacy and safety of AC-IRE ablation. Characterize the cellular and tissue damages induced by the AC-IRE application. Methods: Three study models are employed. In the first one, a cuvette-based system is used to calibrate the optimal pair of electric field-number of pulses to induce complete cell death. In the second one, a microelectrode array (MEA) system is used in combination with calcium imaging to identify electrical and ionic changes in cardiac cells affected by the identified pulse combination. In the third one, a swine model was used to study underwent AC-IRE ablation in left atrium (pulmonary vein ostia, chamber walls) and intraventricular septum.
