O projektu
Počet operací náhrady kolenního kloubu neustále narůstá. Přesto je i v dnešní době stále významným problémem omezená životnost náhrad, kdy je nutné v případě jejich selhání provést revizní operaci. Ta představuje významný zásah do kvality života pacienta. Mezi nejčastější příčiny vedoucí k selhání náhrady patří aseptické uvolnění v důsledku osteolýzy. Jedná se o degradační proces, při kterém dochází k zánětlivé reakci uvolněných otěrových částic a okolní tkáně. Opotřebení artikulujících ploch, které bezprostředně souvisí s procesy tření a mazání, tak představuje z hlediska životnosti náhrady zásadní problém. Při snaze omezit míru otěru a v konečném důsledku tak zlepšit kvalitu života pacientů je proto nutné porozumět tribologickým procesům v kontaktu.
Popis anglickyThe aim of this project is to assess the effect of friction, wear and lubrication on the durability of knee joint replacements. The main attention will be paid to the interaction of the mentioned tribological processes in an effort to extend the implants longevity avoiding the necessity of the need of revising operations. American partner has a long-term experience with the investigation of friction and wear, Czech side will provide a fundamental knowledge about the formation of lubricating film. The obtained data will be confronted with the clinical practice as well. Thus, the present project will provide a complementary knowledge about the tribological processes within the contact. Based on the results, the theory of knee joint replacements lubrication will be defined. So far, such a study has not been performed. The Czech institution will be focused on the observation of lubrication processes in the contact of the replacement components. Lubricant film thickness will be determined while the simultaneous observation of protein aggregations and protein adsorption onto rubbing surfaces will be performed. The main attention will be paid to the effect of load, kinematic conditions and the composition of model synovial fluid on lubrication processes. The US collaborator will initially analyse the composition of the synovial fluid of the patients undergoing total knee replacement. Further, the US researchers will analyse friction and wear of implants. Moreover, the results will be confronted with wear analysis of implants retrieved from the patients during the revising operations, allowing the comparison of laboratory and clinical data. One of the sub-aims of the project is to develop a knee joint simulator enabling to apply kinematic and loading conditions respecting the current standards. Moreover, it will allow to observe the contact in-situ. For this purpose, optical methods based on optical interferometry and fluorescent microscopy will be employed.
